The symptom is that one cannot select more than one state (or county) in a column, and one cannot "unselect" a state in one column to choose one in another. The form seems to allow only one choice per column, but each column can have a marked choice.
Under X windows, simply click the left mouse button to select or unselect any state (or county).
Under MS Windows. use CTRL-CLICK (i.e. hold down CTRL key and click the left mouse button).
On a Mac, use COMMAND-CLICK (i.e. hold down COMMAND key and click the mouse button).
In other words, CTRL-CLICK in MS Windows behaves as COMMAND-CLICK in the Mac and as a simple CLICK in X Windows.
To clear all states, press RESET (next to SUBMIT).
